I consent to UJEP, as the administrator of personal data, keeping and processing my personal data (first name, surname, birth ID number) and appended photograph in the UJEP information system for the purpose of verifying the status of the student in the sense of Act No 111/1998 Coll., on Higher Education Institutions, as amended, and to it doing so for the period of study and for the five years subsequent to its ending. This data will be made available only to the entrusted employees of UJEP and legal persons who on the basis of a contractual relationship with UJEP carry out activities leading to ensuring performance of the subject of activity of UJEP. In the case of an application for an ISIC licence, I agree that UJEP provide my personal data (surname, first name) to the company GTS International, s.r.o., business ID 48114855, for the purpose of records in the international database of holders of valid ISIC ID cards.

Instructions for filling in and submitting application for issue of student ID card.

Fill in your personal data correctly. We also recommend that you fill in your email address for the purposes of communication during the making of the ID card.

Attach a file with a passport photograph in the JPG format with a size of at least 230x296 pixels and maximum of 300x500 pixels. This photograph must correspond to the parameters of a standard passport photograph (single-colour background, 35x45mm, complete head).

Select the type of card. For an additional charge, full-time students can choose a card with an ISIC licence (ISIC international student card), you can find more information about the ISIC card at www.isic.cz.

Price for issue of ID cards (prices for student ID card are according to the order of the rector 7/2012):

Without ISIC licence CZK 150

With ISIC licence CZK 250

Fees are paid at the UJEP cash desk before the student ID card is picked up. When picking up the student ID card, the student presents a confirmation of payment of the fee issued by the UJEP cash desk.
